PHP Structure missing Follow

Hi everyone!

I'm having a weird issue.

I have multiple projects and on all of them when I open structure window.. it's empty. :(

Also, seems like some buttons are missing and menu has a bug...look at the screenshot. (Note the menu !tool.window.move.to.action....)
