Unable to renew student pack.
I have already authorise with GitHub but on that page it's says to fill the details below to apply. But there are no form or field to apply. Please help. I don't have education mail id or isic card. Only have official documents of my college. And GitHub students developer pack.

I'm having the exact same issue. Says to fill out a field but I don't see any.
I got reply from there support. There is some issue with communication with GitHub and they are working on resolving this. So it should be resolved soon until then use the trail version.
