problem with proxy connection window...

When I set the configuration to prompt for a proxy password, I get the proxy connection window that has the login and password but no ok/cancel buttons, I cannot do anything, cannot close the window, cannot hit ok/cancel button, the enter does not work - all I can do is close the window.

I am attaching a gz screen shot - the actual png file turned out to be more than 2MB.
