Debugging not debugging all the time

I've just hit a problem which won't go away nor am I able to figure out.
This happens with both Pallada and Irida. Hopefully someone here can
help me get unstuck.

This is after clearing all caches and creating a project for each
version from scratch.

For some reason when running a web app, in this case it's a quite simple
webapp which depends on one other java module, so there is no web app
interdependency in this case, I can't get the debugger to break or to
even mark the break point as a valid or invalid breakpoint in either my
src files or my jsp files. In src files it sometimes work and others it
doesn't. src is compiled fresh so it's not like I added code which is
causing the breakpoints to not be marked as such.

In the JSPs it's a different bizarre issue... If I have header.jsp,
body.jsp, taglibs.jsp, body includes both header and taglibs.jsp, I can
set a breakpoint in header and taglibs, but I can't set one in body. I
can set it, but it stays red with no marks in it, even after hitting the
page.

I'm at a complete loss. The behavior is the same on Mac and Windows.

Any help would be greatly appreciated.

Tomcat Server (sorry should have specified). I've tried it with Tomcat
5.0.25, 27, 28, 29. All behave the same.

Eugene after further investigation, it seems to be repeatable if I have
a Struts project which uses tiles. The jsp part which uses tiles will
not break no matter what, and the Actions in struts will not stop in
some but not all areas of the source code.
