Capture.PNGOk, this is weird. I went to get you an example and it ONLY occurs in .php files (mixed PHP and HTML.)

This does not throw an Inspection warning:

But this does:

Ah, the issue has nothing to do with PHP. The issue exists when DOCTYPE is missing (it's not, but the IDE can't find it because it's written in PHP.) Filing a bug.
