Can't get debugging to work on live server

I am trying to get debugging with XDebug working on my live server without success. Here's what I have in php.ini:

xdebug.remote_host=[MY IP]
xdebug.profiler_enable = 0
xdebug.profiler_enable_trigger = 1
xdebug.profiler_output_name = "callgrind.out.%t-%s"
xdebug.profiler_output_dir = "/tmp/xdebug_profiler"
xdebug.extended_info = 1
xdebug.idekey = "PHPSTORM"

I'm trying to use zero configuration debugging, so I have the Chrome extension installed and set to debug mode. Debugging works fine on my local development machine (running WampServer), but I'm getting the error shown in the attached image when trying to debug on my live server. I'm running W7 Pro x64 and PhpStorm 7.1.3.
