Workflows don't trigger or take too long to process actions.

I have created a workflow in which the trigger is 'on merge request approval' and the action is 'run pipeline'. However, the workflow is not getting triggered when a merge request is approved. I have also added a delay of 5 minutes in the workflow, but still, the workflow is not getting triggered. Can you please help me troubleshoot this issue?V
