Intellij Idea's PHP xdebug will only break at the first line

Like I said in the title, my Intellij Idea's PHP xdebug will only break at the first line.  After that it will not break at any break points.  If I remove the "Break at the first line" option in my Run/Debug Configurations it will still not hit any break points.  If it's hitting the first line then I assume the xdebug is set up correctly.  I'm not really sure where to go from here.
