Refactor with Shift-F6 using suggested name results in bad code

When renaming a variable by selecting it and pressing Shift-F6 (OSX); If I use the suggestion drop-down and select one of the items and press enter; the result is all screwed up.
I'm in IntelliJ with PHP plugin.
