Debug breakpoints not being hit

Hi - attached are screen shots of how I have my debug configuration set up.  I also have the JetBrains Firefox extension installed.  When I run debug on development a blank browser screen comes up but does not navigate to the address specified.  I enter the address manually and navigate to the page causing the error.  The error displays on the page but the breakpoints I set in the code are never hit.  What am I doing wrong?
