Tim,as I previously pointed out, AppCode doesn't currently have project-configuration UI; to create and configure build targets you still need to use Xcode.

Yes, appCode will detect changes in your Xcode project and reload them automatically.Moreover, chages made in appCode will be detected in Xcode and it will offer to reload them too.
