Basic template include not working

So, I understand this question is really basic, but for my life I can't figure out what I'm doing wrong, and I did my best trying to get it to work by myself. I believe the two following images should be self-explanatory: they show a template include that I created, a very simple one, as well as a very simple php file that is supposed to be parsing that template include. As it turns out, however, nothing like that actually happens. Please help!
