Default "PKCS12" (in uppercase) should work. No, you don't need to fill other fields. Can you please attach screenshot of the error in this case?

Popup is visible when IDEA can detect Flash module from context, i.e. when any file from Flash module is open in the editor. Thant's because popup is different for each Flash module.
