Argh, I haven't any good memories about trying this. Not sure how good or bad the UML part is, but their DB Visual Architect is utter crap, at least for models with 20+ tables. Ridiculously slow and extremely buggy. Certainly nothing I'd recommend...
IntelliUML Teresa is a pretty good tool with excellent IDEA integration. It's fairly new so the feature-set isn't quite as great as some of the more established tools but it seems to be getting there and it feels just like part of IDEA.
I still haven't got myself a full copy since I've not been work on projects that use UML for a while (so haven't been able to justify the cost), however I used the original beta version quite a bit which wasn't bad and I believe its come along in leaps and bounds since then.
This looks like what I need. But it is for version 4.5. Any idea when it will be available for 5.0?

Well I just tried it. My rule for evaluating software if I can't get it to work in 5 minutes, it is not for me. I tried several times to create on simple diagram and I failed. I also found spelling mistakes on the messages that did not inspired confidence.

Keep in mind that if you need Java 5.0 support, you will have to wait longer. Beto was not able to provide a date for 5.0 support. Unfortunately, a lot of the modeling tools (except the real pricey ones) do not support 5.0 at this time.

According to Beto Software (the developers) the 5.0 version will be coming out by the end of September, so not too long to wait.

I'm using DiamondSpin. It has an auto code styler and full round-trip engineering. Look at http://www.crystalbyte.com
