I don't know when this stopped happening, but is there a way to turn it on so that Quick Documentation shows always, instead of having to click the ellipse?
I cannot find any proof that there was a feature that made Quick Documentation appear in the Completion list automatically. Are you sure it's ever been the case?
Please note that you don't need to open the submenu with mouse, you can just it Ctrl+Q (F1 for macOS).

I’m positive that I have never had to open the QD window manually.
Can you remember whether it was a floating popup or a docked tool window?
Floating. Looked almost exactly like this, but I came up automatically:
File | Settings | Editor | General | Code Completion: Show the documentation popup in .... ms checkbox.
