Even if the Environment variables are not shown in the settings and don't work on Tools -> Vagrant after restart of PhpStorm, the .idea/vagrant.xml file contains them. PhpStorm just doesn't use them.
I hope this is the last one: "include system environment variables" doesn't work at all. As soon as I re-open the settings, the checkbox is empty again. At least the manual set ones stay until restart.
Thanks for describing the problem. I can't see similar reports on our tracker but I can reproduce this both in stable and in EAP PhpStorm versions. It would be great if you could submit this to our tracker at https://youtrack.jetbrains.com/newIssue in same details. You'll be getting answers directly from a responsible team after that.
I've testet some more:
Open the Vagrant settings
Click on the icon in the Environment variables input field
Set the checkbox to include system environment variables.
Close the dialog with click on OK, close the settings.
Re-open the Vagrant settings, click on the Environment variables icon again. The before set checkbox is gone.
